Pride Home Care Corp, located at 6851 10th Ave SW Suite A, Rochester, MN 55902, is a licensed provider of Home and Community-Based Services (HCBS) under Minnesota Statute 245D. Our mission is to support individuals with disabilities and older adults in their homes and communities with dignity, respect, and compassion. All services are delivered in compliance with the Minnesota Department of Human Services (DHS) regulations.
We are committed to providing culturally sensitive care that respects the diverse backgrounds, languages, and values of the clients and families we serve. Our team emphasizes cross-cultural understanding to enhance the effectiveness of our care.
We utilize digital links for clients, legal representatives, and case managers to sign necessary forms. These secure links include instructions and can be sent via email or text. Staff use an EVV (Electronic Visit Verification) system to clock in and out only at the client’s home. Clients confirm service hours using a personal PIN, ensuring secure and accurate time tracking. Timesheets must be signed by both client and staff through the EVV app.

Pride Home Care Corp complies with Minnesota Statutes Chapter 245C, which requires comprehensive background studies for all staff before employment. Our process includes:
• DHS background study via NETStudy 2.0
• Valid government-issued photo ID
• Social Security Number provided securely via encrypted email or fax
• Fingerprinting and photography as required for FBI checks
All personal data is handled securely and confidentially, in accordance with DHS standards.
